Dispensing of Emergency Hormonal Contraceptive Pills, Pregnancy Test Kits and Pharmacy Service for Sexually Transmitted Infections by Drug Stores in Chon Buri Province

Abstract

การวิจัยเชิงปฏิบัตินี้มีวัตถุประสงค์เพื่อศึกษาบริการด้านผลิตภัณฑ์สุขภาพทางเพศแลพอนามัยเจริญพันธุ์ ในร้านยา โดยการสำรวจและจัดทำข้อเสนอเชิงนโยบายเพื่อเสริมสมรรถนะร้านยาให้เข้าร่วมเป็นเครือข่ายบริการอนามัยเจริญพันธุ์และป้องกันโรคเอดส์ ในช่วงระยะเวลาตั้งแต่ พ.ศ. 2552 ถุง พ.ศ. 2553 ผู้ตอบแบบสอบถาม 441 คนจากร้านยาทั้งหมด 634 แห่ง (70%) ในจังหวัดชลบุรี มียอดจำหน่ายยาคุมกำเนิดฉุกเฉินจำนวนมากถึง 16,870 กล่อง/เดือน ถุงยางอนามัย 14,883 กล่อง/เดือน และชุดตรวจการตั้งครรภ์ 6,943 กล่อง/เดือน ร้านยาที่อยู่ใกล้สถานศึกษามีการจำหน่าย ยาคุมกำเนิดฉุกเฉิน ถุงยางอนามัย แลละชุดตรวจการตั้งครรภ์ มากกว่าร้ายยาที่อยู่ไกลสถานศึกษาอย่างมีนัยสำคัญทางสถิติ (p < 0.05) ผู้รับบริการรักษาโรคติดต่อทางเพศสัมพันธ์ที่ซื้อยากินเองจากคำโฆษณามีจำนวน 5,168 คน/เดือน มากกว่าผู้รับบริการที่บอกเล่าอาการให้ร่นยาจัดยาให้จำนวน 2,444 คน/เดือน ยาที่สั่งจ่ายเพื่อต้านเชื้อโรคติดต่อทางเพศสัมพันธ์ หนองในแท้ หนองในเทียม แผลริมอ่อน ส่วนใหญ่อยู่ในกลุ่ม Quinolone 74.2% ได้แก่ Ciprofloxacin และ Norfloxacin ยากลุ่มอื่นได้แก่ Doxycyclin 45.7% Azithromycin 26.5% และอื่นๆ อีกถึง 19 ชนิด ร้านยาส่วนใหญ่มีบริการให้ความรู้เรื่องโรคและการใช้ยา แต่บริการให้คำปรึกษาเฉพาะด้านอนามัยเจริญพันธุ์ยังมีน้อย ร้านยา 358 แห่ง (89%) ยินดีเข้าร่วมเป็นเครือข่าย โดยส่วนใหญ่ต้องการให้มรการอบรม (94%) และสนับสนุนเอกสารเผยแพร่ (99.5%) ผลจากการจัดทำข้อเสนอระดับนโยบาย สำนักงานสาธารณะสุขจังหวัดชลบุรีได้จัดให้มีการอบรมเสริมสมรรรถนะร้านยาด้านการให้คำปรึกษาด้านเอดส์และโรคติดต่อทางเพศสัมพันธ์และส่งเสริมการเข้าถึงถุงยางอนามัยและสร้างเครือข่ายที่ครอบคลุมทั้งด้านสุขภาพและสังคม The objectives of this operation research were to study the sexual and reproductive health services of drug stores in providing the preventive and therapeutic cares for the STI/HIV, pregnancy test, condom supply, Family planning as well as referral linkage with the related health services for youth clienteles. Questionnaire were distributed to get the information from 441 of the 63 (70%)drug stores in Chon Buri during 2009-2010. A large number, (16,870) of emergency hormonal contraceptive pills (EHC) was reported to be sold each month, and 14,883 condoms and 6,943 pregnancy test kits were distributed. The supply of EHC, condoms and pregnancy test kits by drug stores located near schools/academic institutions was significantly higher than those far away  (p < 0.05). STI services were reported to be 2,444 clienteles/ month. Each month 5,168 customers bought medicine from  advertisement. The variation of 19 antibiotics was dispensed for gonorrhea, non-gonococcal  urethritis, and chancroid, in which 74.2% was in the quinolone group (such as ciprofloxacin and norfloxacin), 45.7% was doxycyclin, and 26.5% was azithromycin, and atc. Most of the drug stores provided general information about medicine and treatment but less advice on prevention and reproductive health. There were 358 drug stores Z89.1%X willing to  co-operate in the reproductive health service network. Training was needed by 94.0%.  and 99.5% asked foe information leaflets. As a result of the executive report by the authors, the provincial  public health office had drug stores trained in the reproductive health service. The condom supplied through drug stores was the strategy to increase the accessibility of condoms for the prevention of HIV/ STD.
