Abstract

ในอดีตอนามัยเจริญพันธุ์มักเน้นการดูแลสุขภาพของสตรีวัยรุ่นรวมถึงภาวะการมีประจำเดือน การตั้ง ครรภ์ และโรคติดต่อทางเพศสัมพันธ์ แต่เมื่อสตรีเหล่านี้เข้าสู่วัยทอง วงการแพทย์และสาธารณสุขให้ความสำคัญกับมิติด้านสังคมและวัฒนธรรมน้อยกว่ามิติด้านอื่น ๆ เพราะการให้ความหมายของสตรีวัยทองมีความแตกต่างกันทั้งในมุมมองทางการแพทย์ ชีววิทยา สังคม มานุษยวิทยา และจิตวิทยา สิ่งเหล่านี้ส่งผลต่อการดูแลสุขภาพอนามัยเจริญพันธุ์ในสตรีวัยทอง บทความนี้มีวัตถุประสงค์เพื่อสร้างแนวคิดใหม่เกี่ยวกับภาวะอนามัยเจริญพันธุ์ในสตรีวัยทอง ปัญหาทางเพศและการใช้ฮอร์โมนทดแทนบำบัดในสตรีวัยทอง รวมถึงข้อเสนอแนะเชิงนโยบาย และงานวิจัยในอนาคตซึ่งควรจะรวมเรื่องดัชนีชี้วัดทางสุขภาพอนามัยและการสนับสนุนทางสังคม โดยใช้การมีส่วนร่วมของชุมชน และการวิจัยระบบสาธารณสุขในท้องถิ่นด้วย  The main aim of this article was to reconstruct the concept of Menopause, sexual problem and hormonal replacement therapy in menopause including making recommendations for policy and research. In the past, reproductive health focused on female adolescent health care including menstruation, pregnancy and sexual transmitted disease. The public health system does not pay enough attention to women in premenopausal life. In the pre-menopause period, the importance of social and cultural perspectives is less than other perspectives from the medical and public health system because the meaning of menopause is different in medical, biological, social, anthropological and psychological disciplines. The different meanings affect the healthcare of these populations (menopause) in reproductive health care. In the future, an index of menopausal health should be included such as health status index, as well as social support by participatory action in community and local public health research.
