ผลสัมฤทธิ์ของการบริหารจัดการกองทุนหลักประกันสุขภาพระดับท้องถิ่นของสำนักงานหลักประกันสุขภาพแห่งชาติเขต 5 ราชบุรี.

Abstract

ผลสัมฤทธิ์ของการบริหารจัดการกองทุนหลักประกันสุขภาพระดับท้องถิ่นของสำนักงานหลักประกันสุขภาพแห่งชาติ เขต 5 ราชบุรี อยู่ในระดับสูง ปัจจัยที่มีความสัมพันธ์ต่อผลสัมฤทธิ์ของการบริหารจัดการกองทุนหลักประกันสุขภาพระดับท้องถิ่น พบว่า ปัจจัยด้านภาวะผู้นำ มีความสัมพันธ์มากที่สุด ปัจจัยด้านเศรษฐกิจมีความสัมพันธ์น้อยที่สุด สำหรับการวิเคราะห์แบบถดถอยพหุคูณ และคัดเลือกตัวแปรแบบขั้นตอน (Step-wise Regression) พบว่า มีเพียง 6 ปัจจัยเท่านั้นที่สามารถทำนายความเป็นไปได้ที่มีอิทธิพลต่อผลสัมฤทธิ์ของการบริหารจัดการกองทุนหลักประกันสุขภาพระดับท้องถิ่น คือ 1. อายุ 40 ปีขึ้นไป 2. ทัศนคติ 3. การพัฒนาศักยภาพ 4. ภาวะผู้นำ 5. การสนับสนุนจากหน่วยงานที่เกี่ยวข้อง 6. กฎระเบียบ และเมื่อเปรียบเทียบผลสัมฤทธิ์ของการบริหารจัดการกองทุนหลักประกันสุขภาพระดับท้องถิ่น พบว่า กลุ่ม A+ สูงกว่า กลุ่ม B +, กลุ่ม C และในกลุ่ม A สูงกว่ากลุ่ม B อย่างมีนัยสำคัญทางสถิติ ปัญหา อุปสรรคของการบริหารจัดการกองทุนหลักประกันสุขภาพระดับท้องถิ่น คือ ด้านความรู้ ด้านเศรษฐกิจ และด้านกฏระเบียบ ส่วนแนวทางการพัฒนา ส่งเสริมการบริหารจัดการกองทุนหลักประกันสุขภาพระดับท้องถิ่นที่สำคัญ คือ การพัฒนาความรู้ ทัศนคติ การพัมนาศักยภาพการวิจัยได้นำมาสู่ข้อเสนอแนะ คือ การให้ความสำคัญการมีระบบ บริหารจัดการที่มีประสิทธิภาพ โปร่งใส ตรวจสอบได้ และสร้างความเป็นเจ้าของ (Ownership) ของทุกภาพส่วนที่เกี่ยวข้องในชุมชน และควรมีการจัดการทำหลักสูตรการพัฒนาเพิ่มขีดความสามารถ ด้านการบริหารจัดการ หลักสูตรการพัฒนาทักษะภาวะผู้นำ เป็นต้นThe result reveals that an Achievement of Administration on Local Health Security Found of National Health Security Office Region 5 Ratchaburi is of a high level. The factors which most relate to an achievement of Local Health Fundmanagement is leadership whereas the factor which least related to an achievement of Local Health Fund management is economic factor. In multiple regression analysis and step-wise regression found that, there are only six factors which can predicted the possibility influence an achievement of Local Health Fund management. There are age of older than forty years old, attitude, potential development, leadership, related organization's support and rules. The comparison of an achievement of Local Health Fund management classified as an evaluation of Local Health Fund management shows that an achievement of Local Health Fund management, group A+ is higher than group B and C. group A is higher than group B in statistical significance.The results of studying in qualitative research displayed that problems and obstacles in Local Health Fund management are knowledge, economic and rules and the promoting of local Health Fund management that influences an achievement of Local Health Fund management ordered in magnitude are as follow: 1. knowledge 2. attitude 3. potential development etc. This research leads to these following proposals: policy proposal able to scrutinize management is important and conduce to ownership in all sections of community and Establishing the management potential development course, leadership development course.
