การพัฒนาระบบบริการสุขภาพของโรงพยาบาลรัฐเพื่อความมั่นคงทางสุขภาพของประชาชน : ศึกษากรณีเครือข่ายบริการสุขภาพจังหวัดชลบุรี

Abstract

         การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อวิเคราะห์ระบบบริการสุขภาพประชาชนของโรงพยาบาลรัฐ โดยศึกษาปัญหาและข้อจำกัดในการดำเนินงานด้านระบบบริการสุขภาพประชาชนของโรงพยาบาลรัฐ และเสนอแนวทางการพัฒนาระบบบริการสุขภาพของโรงพยาบาลรัฐที่ตอบสนอง ต่อความมั่นคงทางสุขภาพของประชาชน เครื่องมือที่ใช้ในการวิจัย คือ แบบวิเคราะห์เอกสารและกึ่งโครงสร้างแบบสัมภาษณ์ วิเคราะห์ข้อมูลโดยการวิเคราะห์เนื้อหา เพื่อหาข้อสรุปตามวัตถุประสงค์การวิจัย         ผลการวิจัยพบว่า 1) ระบบบริการสุขภาพประชาชนของโรงพยาบาลรัฐเป็นการบริการทางการแพทย์ที่มีกระบวนการสำคัญ 4 มิติ ได้แก่ การสร้างเสริมสุขภาพ การป้องกันโรค การรักษาและการฟื้นฟูสมรรถภาพ มุ่งเน้นการปฏิบัติงานเชิงรุกแบบมีส่วนร่วมกับภาครัฐ ภาคเอกชนและภาคประชาชน สามารถเข้าถึงบริการได้อย่างไม่เป็นอุปสรรคและครอบคลุมทุกกลุ่มวัย 2) ปัญหาและข้อจำกัดในการดำเนินงานด้านระบบบริการสุขภาพประชาชนของโรงพยาบาลรัฐในเครือข่ายบริการสุขภาพจังหวัดชลบุรี ได้แก่ การให้การศึกษาและฝึกอบรมด้านการสร้างเสริมสุขภาพอย่างครอบคลุมทุกกลุ่มวัย การทำงานเป็นทีมด้านการป้องกันโรคร่วมกับทุกภาคส่วน การติดต่อสื่อสาร ที่มีประสิทธิภาพเพื่อการส่งต่อด้านการรักษา และเครื่องมือหรือเครื่องจักรด้านการฟื้นฟูสมรรถภาพไม่เพียงพอสำหรับการดูแลต่อเนื่องที่บ้าน 3) แนวทางการพัฒนาระบบบริการสุขภาพของโรงพยาบาลรัฐที่ตอบสนองต่อความมั่นคงทางสุขภาพของประชาชน ได้แก่ การกำหนดแนวทางการให้การศึกษาและฝึกอบรมด้านการสร้างเสริมสุขภาพสำหรับทุกกลุ่มวัย แนวทางการติดต่อสื่อสารที่มีประสิทธิภาพเพื่อการส่งต่อด้านการรักษา และแนวทางการจัดการด้านการฟื้นฟู สมรรถภาพต่อเนื่องที่บ้าน การจัดทำกลยุทธ์ด้านการป้องกันโรคติดต่อและโรคไม่ติดต่อเชิงรุก โดยสนับสนุนการมีส่วนร่วมกับภาครัฐ ภาคเอกชน และภาคประชาชน นอกจากนี้ สำนักงานสาธารณสุขจังหวัดชลบุรีควรเสนอให้กระทรวงสาธารณสุขเพิ่มการสนับสนุนเงินทุน แยกค่าบริการสุขภาพและเงินเดือนข้าราชการ และปรับปรุงสิทธิสุขภาพให้เป็นแบบร่วมจ่าย ทั้งนี้ ผู้นำหรือผู้บริหารระดับสูงของโรงพยาบาลรัฐในเครือข่ายบริการสุขภาพจังหวัดชลบุรี เป็นผู้ที่มีบทบาทสำคัญเกี่ยวกับการทำงานเป็นทีม การติดต่อสื่อสารที่มีประสิทธิภาพ และการจัดทำกลยุทธ์ร่วมกับภาครัฐ ภาคเอกชนและภาคประชาชน             The purpose of this research aims to analyze the government hospital health service system for people, study the problems and limitations of the government hospital health service system for people and propose way to develop the government hospital health service system for people’s health security. The research instruments were semi-structure interview and content analysis based on research objectives.           Research result: finding 1) the government hospital health service system for people is a four-step medical service includes health promotion, disease prevention, treatment and rehabilitation. Focus on proactive operation and participates with government, private sector and people. Access to service Unobstructed and cover all age groups. 2) The problems and limitations of the government hospital health service system for people in Chonburi provincial health service network including education and training on health promotion covering all age groups, Team work on disease prevention with all sectors, Effective communication for treatment transitions and Insufficient rehabilitation equipment or Machinery for home care. 3) The way to develop the government hospital health service system for people's health security including setting guidelines for education and training on health promotion for all age groups, effective communication for treatment transitions and continuing rehabilitation management at home, proactive prevention strategies for communicable disease and non-communicable disease, participates with government, private sector and people. In addition, Chonburi provincial health office should be proposed to the ministry of public health increased money support, the separation of health services and government salaries, and improve the right to health as co-pay. The leaders or top managers of the government hospital in Chonburi provincial health service network are the key role in the teamwork, effective communication and strategic planning with public sector, private sector and people.
