การสร้างมาตรฐานการพยาบาลเพื่อดูแลสุขภาพผู้ป่วยเบาหวานที่บ้านสำหรับพยาบาลวิชาชีพที่ปฏิบัติงาน ณ ศูนย์สุขภาพชุมชน

Establishment of Nursing Standards of Home Health Care in Patient with Diabetes Mellitus for Professional Nurse working in Primary Care Units.

Abstract

การศึกษาวิจัยครั้งนี้มีวัตถุประสงค์เพื่อ สร้างมาตรฐานเชิงกระบวนการ และมาตรฐานเชิงผลลัพธ์ของการพยาบาล ในการดูแลสุขภาพผู้ป่วยเบาหวานที่บ้าน สำหรับพยาบาลวิชาชีพที่ปฏิบัติงาน ณ ศูนย์สุขภาพชุมชน กลุ่มตัวอย่างเป็นผู้เชี่ยวชาญ จำนวน 23 คน เลือกกลุ่มตัวอย่างด้วยวิธีการบอกต่อ ดำเนินการวิจัยสร้างมาตรฐานครอบคลุมเนื้อหา ตามขั้นตอนของกระบวนการพยาบาล และใช้เทคนิคเดลฟาย ตรวจสอบความเที่ยงตรงด้านเนื้อหา จำนวน 3 รอบ ผลการวิจัยได้มาตรฐานการพยาบาล เพื่อดูแลสุขภาพผู้ป่วยเบาหวานที่บ้าน สำหรับพยาบาลวิชาชีพที่ปฏิบัติงาน ณ ศูนย์สุขภาพชุมชนจำนวน 11 หัวข้อมาตรฐาน ซึ่งแต่ละหัวข้อมาตรฐานการพยาบาล ประกอบด้วยมาตรฐานเชิงกระบวนการ และมาตรฐานเชิงผลลัพธ์ ดังนี้หัวข้อมาตรฐานที่ 1) การเตรียมความพร้อมของเจ้าหน้าที่ก่อนให้การดูแลสุขภาพ ผู้ป่วยและครอบครัวที่บ้าน 2) การสร้างสัมพันธภาพที่ดีกับผู้ป่วยและครอบครัว 3) การประเมินวินิจฉัยภาวะสุขภาพของผู้ป่วย และประเมินภาวะเสี่ยงต่อการเป็นเบาหวานของบุคคลในครอบครัว 4) การส่งเสริมให้ผู้ป่วยและครอบครัวมีความรู้ความเข้าใจเกี่ยวกับโรคเบาหวาน และภาวะแทรกซ้อนจากโรคเบาหวานอย่างถูกต้อง 5) การส่งเสริมให้ผู้ป่วยและครอบครัว วางแผนการดูแลสุขภาพ และหาแหล่งสนับสนุน เพื่อตอบสนองด้านร่างกาย จิตใจ อารมณ์ และสังคม 6) การส่งเสริมให้ผู้ป่วยและครอบครัวมีความรู้ ความเข้าใจ เกี่ยวกับการควบคุมอาหารอย่างถูกต้อง 7) การส่งเสริมให้ผู้ป่วยและครอบครัวมีความรู้ ความเข้าใจเกี่ยวกับ การออกกำลังกายอย่างถูกต้อง 8) การส่งเสริมให้ผู้ป่วยและครอบครัวมีความรู้ ความเข้าใจ เกี่ยวกับการบริหารยาเม็ดรับประทานอย่างถูกต้อง 9) การส่งเสริมให้ผู้ป่วยและครอบครัวมี ความรู้ความเข้าใจ ในการดูแลสุขภาพโดยทั่วไปและสุขภาพเท้าอย่างถูกต้อง 10) การส่งเสริม ให้ผู้ป่วยและครอบครัวมีความรู้ความเข้าใจ และสามารถหาแนวทางจัดการกับความเครียดได้ ย่างเหมาะสม 11) การประเมินผลการพยาบาล เพื่อให้ผู้ป่วยและครอบครัวสามารถดูแล ตนเองได้อย่างต่อเนื่อง  The purpose of this research was to establish process standards and outcome standards of home health care nursing in patient with diabetes mellitus for professional nurse working in primary care units. Twenty-three specialists were selected by snowballing technique. Nursing process was used in the establishment of nursing standards and the content validity was confirmed by 3 rounds delphi technique.  Eleven nursing standards of home health care in patient with diabetes mellitus for professional nurse in primary care units were generated from the research. Each of nursing standards was composed of process and outcome standards. The eleven standards were as follows : Standard 1 : The staff's preparation of home health care prior to visit of the patients and their family. Standard 2 : The establishment of good relationship with the patients and their family. Standard 3 : The assessment and diagnosis of patients' health and the family member high risk person screening. Standard 4 : The patients and their family promotion of correct knowledge, understanding of diabetes mellitus, and its complications. Standard 5 : The promotion for patients and their family to plan for health and seek social support for physio-psycho-social health. Standard 6 : The patients and their family promotion of knowledge and understanding of correct dietary control. Standard 7 : The patients and their family promotion of knowledge and understanding of correct physical activity. Standard 8 : The patients and their family promotion of knowledge and understanding of correct oral diabetic drug administration. Standard 9 : The patients and their family promotion of knowledge and understanding of correct healthcare and foot care. Standard 10 : The patients and their family promotion of knowledge and understanding of appropriate stress and coping strategies. Standard 11 : The evaluation of nursing care for patients and their family of continuous self-care .
