Nurses' perception on continuing care by having family to be centered of the care

Abstract

          การศึกษาครั้งนี้เป็นการศึกษาที่ใช้ระเบียบวิธีวิจัยเชิงคุณภาพที่มีวัตถุประสงค์เพื่อศึกษาความหมายของการดูแลอย่างต่อเนื่อง การดูแลที่มีครอบครัวเป็นศูนย์กลาง และศึกษาบทบาทของพยาบาลในการดูแลอย่างต่อเนื่องโดยมีครอบครัวเป็นศูนย์กลาง ซึ่งผู้ให้ข้อมูลเป็นพยาบาสวิชาชีพสตรีจํานวน 15 ราย เก็บรวบรวมข้อมูลโดยการทําสนทนากลุ่มและการสัมภาษณ์เจาะลึก วิเคราะห์ข้อมูลโดยใช้การวิเคราะห์เนื้อหา (Content analysis) ผลการศึกษาพบว่า พยาบาลมองการดูแลอย่างต่อเนื่องเป็นการที่ผู้มีความเจ็บป่วย ได้รับการดูแลทั้งทางด้านร่างกาย จิตใจ อารมณ์ และสังคมเศรษฐกิจอย่างต่อเนื่องซึ่งกระบวนการของการดูแล สามารถเกิดขึ้นจากโรงพยาบาลและดําเนินต่อไป สู่บ้านหรือจากบ้านและดําเนินต่อไปยังโรงพยาบาล โดยการดูแลผู้ป่วยที่เกิดขึ้นจะมีการประสานความร่วมมือกันของผู้ดูแล เพื่อส่งเสริมให้การดูแลผู้ป่วยนั้น เป็นไปอย่างปลอดภัยและส่งผลให้ผู้ป่วยสามารถดําเนินชีวิต อยู่ได้ดีที่สุดตามสภาพที่เขาเป็นอยู่ พยาบาลมองการดูแลที่มีครอบครัวเป็นศูนย์กลางว่าเป็นการให้ครอบครัวเข้ามามีส่วนร่วมรับผิดชอบในการดูแลผู้ป่วยทั้งทางด้านร่างกาย จิตใจ อารมณ์ และสังคมเศรษฐกิจ โดยการดูแลผู้ป่วยนั้นปฏิบัติด้วยความถูกต้อง และคํานึงถึงความปลอดภัย ซึ่งส่งผลให้ผู้ป่วยมีกําลังใจ ในการดําเนินชีวิตอย่างมีความสุข และบทบาทพยาบาลที่พบในการดูแลอย่างต่อเนื่องโดยมีครอบครัวเป็นศูนย์กลางแบ่งออกเป็น 6 บทบาท คือ การเป็นผู้ดูแล การเป็นผู้ให้ข้อมูล ความรู้ การเป็นผู้ให้คําปรึกษา การเป็นผู้ช่วยเหลือ การเป็นผู้ประสาน และการเป็นผู้พิทักษ์สิทธิของผู้ป่วย ผลการศึกษาเสนอแนะให้เกิดการพัฒนาอย่างต่อเนื่อง ทางการพยาบาลโดยการสร้างแนวทางในการ ดูแลผู้ป่วยอย่างต่อเนื่องโดยเน้นให้ครอบครัวมีส่วนร่วม และพัฒนาแนวปฏิบัติในการดูแลสมาชิกที่มี ภาวะเจ็บป่วยด้วยโรคต่างๆ ที่เฉพาะเจาะจงอย่างเป็นรูปธรรม เพื่อสนับสนุนให้ครอบครัวสามารถดูแล สมาชิกที่มีภาวะเจ็บป่วยด้วยโรคต่างๆ นั้นได้อย่างสะดวกและปลอดภัย อีกทั้งยังประโยชน์ให้การดูแลผู้ป่วยนั้น ๆ สามารถกระทําได้อย่างมีประสิทธิภาพและประสิทธิผลมากยิ่งขึ้นด้วย This study was using the qualitative method in order to explore nurses' perception on continuing care by having family to be centered of the care. The objectives of this study were to study on meaning of continuing care, family-centered care and to study on nurses' roles related to continuing care by having family to be centered of the care. The participants were fifteen female registered nurses. Focus group and In-depth interview were used as data collection. All data had been analyzed by using content analysis. The finding was found that nurse gave the meaning of continuing care as taking care an illness person in the aspects of physical, psychological, emotional, and socioeconomic with persistently. The process of care would start from hospital to home or home to hospital by collaborating with all caregivers in order to promote safety care for illness person and make her/him carry with her/his life the best as she/he can. Furthermore, nurse participants expressed their meaning of having family to be centered of the care as providing family to join and share their responsibilities to take care of their illness member in the aspects of physical, psychological, emotional, and socioeconomic. The providing care for an illness member would do with understanding, rightness, and safety, which an illness member would have psychological support to go on with her/his life and live with happiness The nurses also explained their roles of care related to continuing care by having family to be centered of the care divided into 6 roles, which were role of care providing, role of providing information and knowledge, role of giving advice, role of assisting, role of collaborating, and role of providing patient's rights. The finding of this study also suggested further study on continuously development on nursing practice due to making clinical nursing practice guideline for nurse to do continuing care by having family to be centered of the care for an illness member and developing guideline for taking care of family member who has illness with any kind of diseases. These further studies would be more beneficial for family to care her/his illness member with any kind of diseases as convenient, safe, and more effective and efficient.
