Developing a model for professional nurses caring for terminally III patients in Banglamung Hospital

Abstract

          การวิจัยเชิงปฏิบัติการครั้งนี้มีวัตถุประสงค์ เพื่อหารูปแบบการดูแลผู้ป่วยระยะสุดท้าย ของพยาบาล  วิชาชีพ โรงพยาบาลบางละมุง เลือกผู้เข้าร่วมวิจัยแบบเจาะจงจากพยาบาลวิชาชีพที่ปฏิบัติงาน ในหอผู้ป่วยหญิง โรงพยาบาลบางละมุง และมีประสบการณ์ในการดูแลผู้ป่วยระยะสุดท้ายอย่างน้อย 1 ราย จํานวน 10 คน ขั้นตอนการศึกษาประกอบด้วย การศึกษาข้อมูลเบื้องต้น เกี่ยวกับการปฏิบัติการพยาบาลในการดูแลผู้ป่วยระยะสุดท้าย ปัจจัยและปัญหาอุปสรรคต่างๆ ในการดูแลผู้ป่วยระยะสุดท้าย หลังจากนั้นเป็นการ แลกเปลี่ยนข้อมูลระหว่างผู้วิจัยและกลุ่มตัวอย่าง ร่วมกันพัฒนารูปแบบการดูแล ผู้ป่วยระยะสุดท้ายทดลองปฏิบัติตามแผนการสังเกต การสะท้อนการปฏิบัติตามแผน และการปรับปรุงแผนเพื่อทดลอง ปฏิบัติใหม่จนได้รูปแบบที่สามารถใช้เป็นแนวทางในการดูแลผู้ป่วยระยะสุดท้ายได้ การรวบรวมข้อมูลใช้วิธีการสัมภาษณ์แบบเจาะลึก การสนทนากลุ่ม การสังเกต และการบันทึกภาคสนาม การวิเคราะห์ข้อมูลโดยการจัดหมวดหมู่ข้อมูล การตีความ และการสร้างข้อสรุป ผลการวิจัยพบว่า รูปแบบการดูแลผู้ป่วยระยะสุดท้ายของพยาบาลวิชาชีพ โรงพยาบาลบางละมุง ประกอบด้วย 1) การสร้างความเข้าใจและการยอมรับ ของผู้ป่วยและญาติเกี่ยวกับภาวะความเจ็บป่วยของผู้ป่วย 2) การส่งเสริมการมีส่วนร่วมของญาติ ในการดูแลผู้ป่วย 3) การเตรียมความพร้อมเพื่อให้ผู้ป่วยเข้าสู่วาระสุดท้ายของชีวิตอย่างสงบและสมศักดิ์ศรี  The objective of this action research was to develop a model for professional nurses caring for terminally ill patients in Banglamung Hospital. Ten professional nurses who were working at female ward of Banglamung Hospital and had prior experience nursing at least one terminally ill patient recruited to participate in this study. The research consisted of 1) base-line survey to identify the strengths and weaknesses relating to caring for terminally ill patients 2) mutual collaboration among the professional nurses and researchers to develop a model for caring for terminally ill patients  3) implementing the model 4) observing and reflecting on the effectiveness the model and 5) continually revising and re-implementing the model until it was satisfactory. In-depth interviews, focus group, participant observations and field notes were carried out to collect data. Data were analyzed by coding, interpreting and developing themes.  The results indicated that the model consisted of 1) transitional process of patients' s and relatives’s acceptance of terminally ill. 2) promoting relatives to participate in caring for terminally ill patients. 3) preparing terminally ill patients coping with death peaceably.
