Factors affecting quality of life among Thai amputees

Abstract

การสูญเสียแขนและ/หรือขาถือเป็นภาวะวิกฤติต่อการดําเนินชีวิตของบุคคลเนื่องจากมี ผลกระทบโดยตรงต่อภาวะสุขภาพทั้งด้านร่างกาย จิตใจ สังคม รวมถึงการดําเนินชีวิตอย่างมีคุณภาพ งานวิจัยนี้มีจุดมุ่งหมายเพื่อศึกษาปัจจัยที่มีอิทธิพลต่อคุณภาพชีวิตของผู้พิการไทยที่สูญเสียแขนและ/หรือขา โดยศึกษากลุ่มตัวอย่างที่สูญเสียแขนและ/หรือขาที่เกิดจากการประสบอุบัติเหตุ จํานวน 187 ราย ที่ได้เข้ารับการฟื้นฟูสมรรถภาพ ณ โรงเรียนอาชีวพระมหาไถ่พัทยา จ.ชลบุรี ศูนย์บริการสวัสดิการทางสังคมเฉลิมพระชนมพรรษา 5 รอบ จ.ลพบุรี ศูนย์ฟื้นฟูสมรรถภาพคนงาน บางพูน จ.ปทุมธานี ศูนย์ฟื้นฟูสมรรถภาพคนงานประจําภาคตะวันออก จ.ระยอง ศูนย์สิรินธรเพื่อการฟื้นฟูสมรรถภาพทางการแพทย์แห่งชาติ จ.นนทบุรี และองค์การทหารผ่านศึก กรุงเทพฯ เก็บรวบรวมข้อมูลด้วยแบบสอบถามส่วนบุคคล แบบประเมินระดับการสูญเสียสมรรถภาพของร่างกาย แบบวัดการสนับสนุนทางสังคมและแบบวัดคุณภาพชีวิต ตั้งแต่เดือนมิถุนายน ถึงเดือนสิงหาคม 2547 วิเคราะห์ข้อมูลด้วยค่าสถิติเชิงพรรณนา ค่าสัมประสิทธิ์สหสัมพันธ์เพียร์สัน และการวิเคราะห์การถดถอยพหุคูณแบบขั้นตอน ผลการศึกษาพบว่ากลุ่มตัวอย่างส่วนใหญ่เป็นชาย (80.70%) ช่วงอายุ 21-25 ปี (22.50%) การศึกษาอยู่ในระดับมัธยมศึกษา (45.50%) มีรายได้ไม่เพียงพอต่อการใช้จ่าย (47.10%) การสนับสนุนทางสังคม โดยรวมของผู้ที่สูญเสียแขนและ/หรือขาอยู่ในระดับดี แต่การช่วยเหลือด้านการเงิน สิ่งของแรงงานและบริการกับการช่วยเหลือด้านข้อมูลข่าวสาร อยู่ในระดับปานกลาง คุณภาพชีวิตอยู่ในระดับดี แต่คะแนนของคุณภาพชีวิตในมิติด้านสังคมและ เศรษฐกิจอยู่ในระดับปานกลาง และพบว่าการสนับสนุนทางสังคม ระดับการศึกษาและระยะเวลาที่สูญเสียแขนและ/หรือขาจนถึงปัจจุบัน เป็นปัจจัยที่สามารถร่วมทำนายคุณภาพชีวิตของผู้ที่สูญเสียแขนและ/หรือขาได้อย่างมีนัยสําคัญทางสถิติโดย สามารถทำนายได้ร้อยละ 38.2 (p<.01)  ผลการศึกษาแสดงให้เห็นว่าในการส่งเสริมคุณภาพชีวิตของผู้พิการไทยที่สูญเสียแขนและ/หรือขานั้นสิ่งสําคัญที่จะต้องคํานึงถึงคือ การสนับสนุนทางสังคม หากได้รับการสนับสนุนทางสังคมที่ดี แล้วผู้ที่สูญเสียแขนและ/หรือขาจะสามารถปรับตัวเข้ากับการเปลี่ยนแปลงของสิ่งแวดล้อม และสามารถเปลี่ยนผ่านสู่ความผาสุกในชีวิตและมีคุณภาพชีวิตที่ดีได้  Having a limb amputated provokes a violent crisis in human lives. This condition may affect their quality of life (QOL) including the physical, psychological and social dimensions. The purpose of this study was to examine factors influencing quality of life among Thai amputees. Participants were 187 Thai amputees with limb loss caused by accident and had been receiving rehabilitation services at the Redemptorist Vocational School for Disabled,  at Pattaya, Chonburi Province, The Labourer Rehabilitation Center, at Bangpoon, Patumtani  Province, Labourer Rehabilitation East Center, Rayong Province, The Social Security Service  Center Chalermprageart, Lopburi Province, Sirindhon Medical-rehabilitation Center, Nonthaburi Province, and The War Veterans Organization of Thailand, Bangkok. Data were collected from June to August 2004 by using a demographic data record form, the Social Support Questionnaire and the Quality of Life Questionnaire. Descriptive statistics and Stepwise Multiple Regression Analysis were employed to analyze the data. The results revealed that the majority of participants were male (80.70 %) whose  age ranged from 21-25 years (22.50%). About half of the participants (45.50%) finished high school. Forty seven percent of participants had very little monthly income and experienced an insufficiency of funds to cover living expenses. The participants reported a slightly  high score on quality of life and social support ; however, the score on social and economic dimension was at a moderate level. The influences of social support together with education level and timing level after limb loss were able to predict 38.2% of the variation of quality of life among Thai amputees (p < 0.01).  Findings from the study demonstrate that providing social support and education about occupation should be focused on enhancing quality of life among Thai amputees. As the amputees get more support and education, they will be able to transit themselves to earn more quality of life.
