กระบวนการปกป้องสิทธิผู้ป่วยของพยาบาลวิชาชีพ

Abstract

การศึกษาครั้งนี้เป็นการวิจัยเชิงคุณภาพแบบทฤษฎีพื้นฐาน โดยมีวัตถุประสงค์เพื่อศึกษากระบวนการปกป้องสิทธิผู้ป่วยของพยาบาลวิชาชีพ ผู้ให้ข้อมูลเป็นพยาบาลวิชาชีพที่ปฏิบัติการพยาบาลในโรงพยาบาลของจังหวัดระยองทั้งหมดจำนวน 20 คน เก็บรวบรวมข้อมูลโดยการสัมภาษณ์เชิงลึก การสังเกต และการวิเคราะห์บันทึกทางการพยาบาล วิเคราะห์ข้อมูลตามแนวคิดของ Straussand Corbin (1998) ผลการศึกษาพบว่า กระบวนการปกป้องของพยาบาลวิชาชีพเกิดขึ้นเป็นลำดับต่อเนื่อง คือ ระยะที่ 1ทำตามและเรียนรู้ พยาบาลวิชาชีพเริ่มเข้าสู่วิชาชีพการพยาบาลให้การดูแลผู้ป่วยตามสิทธิพื้นฐานและการเรียนรู้การปกป้องสิทธิผู้ป่วยจากพยาบาลวิชาชีพที่มีประสบการณ์ระยะที่ 2 อึดอัด-ขัดแย้ง พยาบาลวิชาชีพถึงแม้มีประสบการณ์มากขึ้นแต่เมื่อไม่สามารถปกป้องสิทธิผู้ป่วยได้ เกิดความรู้สึกไม่สบายใจ ลำบากใจ เป็นห่วงผู้ป่วย ส่วนระยะที่ 3 ทุ่มเทอย่างสุดความสามารถ พยาบาลวิชาชีพพยายามอย่างสุดความสามารถโดยพยายามจัดการด้วยตนเองใช้พลังของทีม และผสมผสานกลยุทธ์ที่หลากหลายเพื่อปกป้องผู้ป่วย นอกจากนี้ยังพบว่า เงื่อนไขที่สำคัญได้แก่การรับรู้เกี่ยวกับการปกป้องสิทธิผู้ป่วย การมีความเชี่ยวชาญเชิงวิชาชีพ การมีพลังในตนเอง และมีแหล่งสนับสนุนช่วยให้พยาบาลวิชาชีพปกป้องผู้ป่วยได้ ซึ่งทำให้เกิดผลที่ตามมา คือ ผู้ป่วยได้รับการดูแลตามสิทธิพื้นฐานได้รับการดูแลอย่างปลอดภัย รวมทั้งรู้สิทธิของตนเองและรักษาสิทธิให้กับตนเองได้ผลการวิจัยที่ได้สามารถนำมาใช้เป็นข้อมูลพื้นฐานในการพัฒนาความสามารถการปกป้องสิทธิผู้ป่วยของพยาบาลวิชาชีพ นอกจากนี้ยังช่วยให้ผู้บริหารทางการพยาบาลสามารถนำเงื่อนไขที่เกี่ยวข้องมาใช้สนับสนุนพยาบาลวิชาชีพในกระบวนการปกป้องสิทธิผู้ป่วย ซึ่งส่งผลให้เกิดประโยชน์สูงสุดแก่ผู้ป่วยThe purpose of this grounded theory studywas to describe the process of advocatingpatients’ rights of professional nurses. This studywas conducted in 20 professional nurses, whoworked in hospital at Rayong province. In-depthinterview, observation, and analyze nursing recordswere the strategies used in data collection. Straussand Corbin’s (1998) approach was used to analyzethe data.The result showed that the process ofadvocating patients’ rights of professionalnurses composes of three sequential phases; 1)Following and learning is the early phase for thebeginner nurses. They just do following basicpatients’ rights and learn from experience nurses.2) The feeling of uncomforting and conflict; professional nurses gain more experience. Theyfeel uncomfortable when they still are unable toadvocate patients’ rights. 3) Dedicating to thebest own ability is the last phase; professionalnurses see something that are not right or wrongmorally, they just take actions by themselves,or uses power of team and multi-strategies touphold to advocate patients’ rights. Perception,professional expert, self-power, and support wereconditions to advocate patients’ rights. Theconsequence of this process was that the patientsreceive the standard treatments safely, includingrealizing their rights and protecting themselves.The finding of this study can be usedas the basis to develop competencies ofprofessional nurses to advocate patients’ rights.In addition, nursing administrator can useconditions to support professional nurses toadvocate patients’ rights. This will promote thebest benefit of the patients.
