Abstract

บริการปฐมภูมิ (Primary Care) และการสาธารณสุขมูลฐาน (Primary Health Care) มีความหมายที่ใกล้เคียงกัน บางครั้งพบคำสองคำนี้ใช้แทนกัน แต่โดยแท้จริงแล้วสองคำนี้มีแนวคิดที่แตกต่างกัน มีการรับรู้อย่างแพร่หลายในระดับนานาชาติที่อธิบายความแตกต่างของสองคำนี้  ในบริบทของประเทศไทย บริการปฐมภูมิ (Primary Care) เป็นบริการด่านแรกของระบบบริการสุขภาพที่จัดให้ประชาชนเข้าถึงอย่างถ้วนหน้า โดยมีการเชื่อมโยงกับบริการสุขภาพระดับอื่นผ่านการส่งต่อ และเชื่อมโยงกับภาคส่วนอื่นด้วยความร่วมมือ นอกจากนี้ยังเป็นการจัดบริการทั้งในสถานบริการและในชุมชน ตามคุณลักษณะสำคัญของบริการปฐมภูมิ ส่วนการสาธารณสุขมูลฐานคือแนวคิดที่เกี่ยวข้องกับนโยบายสุขภาพและการจัดบริการ ทั้งในระดับบุคคลและกลุ่มประชากร มีความสัมพันธ์กับการดูแลผู้รับบริการและการสาธารณสุข และสอดคล้องกับองค์การอนามัยโลกที่กล่าวในปี ค.ศ. 2010 ว่าบริการปฐมภูมิเป็นองค์ประกอบหนึ่งของการสาธารณสุขมูลฐาน สำหรับประเทศไทยสถานบริการที่นำแนวคิดการสาธารณสุขมูลฐานนาสู่การปฏิบัติ คือโรงพยาบาลส่งเสริมสุขภาพตำบล หรือสถานีอนามัย ดังนั้น ในปัจจุบันบริการปฐมภูมิและการสาธารณสุขมูลฐาน มีพัฒนาการโน้มเข้าหากัน แสดงให้เห็นถึงความเชื่อมโยงกัน ของทั้งสองคำนี้ ตามที่ประกาศไว้ที่อัลมา อตาและมุ่งไปสู่ความเป็นธรรมที่มากขึ้น  The definitions of primary care and primary health care are very similar, which are often employed interchangeably in meaning. However, these two forms have different concepts which are well-known widely internationally of their distinct interpretation.  In Thailand, primary care is the first-line contact of care within the national health system that provides to all Thais with universal access to health services. Primary care is connected to other levels of care through the referral system and functions in coordination with other sectors. Moreover, primary care provides services both in the health-care facility and in the community according to its attributes. In contrast, primary health care is the concept which is related to health policy and service provision at both individual and population group levels; and it is also related to primary care for individual patients and public health services. This definition is consistent with what the World Health Organization (2010) described, i.e. primary care is a component of primary health care. In Thailand, the health-care facilities that translate the concept of primary health care into practice are sub-district health promoting hospitals or health centers, Therefore, at present primary care and primary health care have a tendency to merge, reflecting their linkages according to the Alma-Ata Declaration in achieving equity in health.
