Best Practice for Primary Care Unit Suitable for Community Context and Demand

Abstract

การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาความต้องการของประชาชนเกี่ยวกับให้การบริการสุขภาพ โดยศึกษา หารูปแบบศูนย์สุขภาพชุมชนที่เหมาะสมสอดคล้องกับปัญหาของพื้นที่ และปัจจัยนำเข้าที่จำเป็นในการพัฒนาการให้บริการของศูนย์สุขภาพชุมชนให้มีประสิทธิภาพ ศึกษาในผู้ที่มาใช้บริการในศูนย์สุขภาพชุมชนที่มีคะแนนมาตรฐานบริหารจัดการสูงสุดในจังหวัดพิษณุโลก สุโขทัย และพิจิตร จังหวัดละ 1 ศูนย์ จำนวน 1,432 คน โดยใช้แบบสัมภาษณ์ในการเก็บข้อมูล และศึกษารูปแบบศูนย์สุขภาพชุมชนต้นแบบในคณะกรรมการสุขภาพ ระดับตำบลทุกคนในศูนย์ที่มีคะแนนมาตรฐานบริหารจัดการสูงสุดรวมจำนวน 95 คน ผลการศึกษาพบว่า ความต้องการบริการทางด้านสุขภาพที่มีค่าเฉลี่ยสูงสุด คือ การให้ความรู้ในการดูแลตนเองเมื่อเจ็บป่วยด้วยปัญหาสุขภาพที่พบบ่อย ส่วนรูปแบบศูนย์สุขภาพชุมชนต้นแบบนั้น คือ ศูนย์ที่มุ่งเน้นงานบริการสาธารณสุข แบบผสมผสานที่สอดคล้องกับความจำเป็นพื้นฐานทางด้านสุขภาพของประชาชนในพื้นที่โดยเฉพาะ ได้แก่ การให้ความรู้ในการดูแลตนเองเมื่อเจ็บป่วยด้วยปัญหาสุขภาพที่พบบ่อย การดำเนินงานสร้างสุขภาพที่สอดคล้องกับวิถีชีวิตของประชาชนในท้องถิ่น บุคลากรและประชาชนมีแนวคิดร่วมในการพัฒนาสุขภาพชุมชนมีเครือข่าย พัฒนาสุขภาพชุมชนร่วมดำเนินงานกับศูนย์สุขภาพชุมชนการปฏิบัติงานบนพื้นฐานข้อมูลสุขภาพชุมชนองค์กร ชุมชนที่อยู่ในพื้นที่มีการบูรณาการในการพัฒนาระบบบริการสุขภาพมีทีมสหวิชาชีพในการให้บริการทั้งในสถานบริการและชุมชน ปัจจัยนำเข้าของศูนย์สุขภาพชุมชนต้นแบบที่สำคัญ ได้แก่ การมีทีมสหวิชาชีพ ระบบข่าวสารครบถ้วนเป็นปัจจุบัน มีเครือข่ายการทำงาน และมีความพร้อมด้านงบประมาณ  The objectives of this research were to describe the basic health demands of the population and find out the best primary care unit practices that were appropriate in solving local problem and studying input factors for the effective primary care units. The population consisted of patients who visited (consulted), the primary care units in Sukhothai, Phitsanulok, and Pichit Provinces as well as health broad of primary care units. The data collection were done using interviews and questionnaires. According to the results, the basic health demands of the population in the highest score was to educated about health care and basic health problem solving. The best primary care unit practices that were appropriate in solving local problems emphasized the service and establishing health promotion programs that are appropriate in health need and solving local community problems. Health care providers and community members started working together by the same vision; building community participation in health care; and establishing a health care networking committee. Establishing a health partnership in the community was a key for success for the primary care model. The multi health personal are working at PCU and community. The input factors of the best practice model were: enough personnel, professional development, effective information management, working as a network and available budget.
