การศึกษาและวิเคราะห์รูปแบบบริการคลินิกนอกเวลาราชการเพื่อพัฒนาคลินิกพิเศษเฉพาะทางนอกเวลา ราชการของหน่วยบริการในสังกัดกระทรวงสาธารณสุข

Abstract

          การศึกษานี้เป็นการศึกษาเชิงพรรณนา ใช้ระเบียบวิธีวิจัยเชิงคุณภาพ มีวัตถุประสงค์เพื่อศึกษาและ วิเคราะห์รูปแบบการจัดบริการนอกเวลาราชการของหน่วยบริการสาธารณสุขทั้งในและนอกสังกัดกระทรวง สาธารณสุขรวมทั้งกฎหมาย ระเบียบ ข้อบังคับต่าง ๆ ที่เกี่ยวข้อง เพื่อจัดทำเป็นข้อเสนอแนะเชิงนโยบายใน การพัฒนาระบบบริการนอกเวลาราชการ และใช้เป็นข้อมูลในการจัดทำระเบียบกระทรวงสาธารณสุข โดย วิธีการเก็บข้อมูลในระดับทุติยภูมิจากงานวิจัย รายงานการศึกษารูปแบบการดำเนินการคลินิกนอกเวลาใน หน่วยบริการ ทบทวนกฎหมายระเบียบ ข้อบังคับต่าง ๆ ที่เกี่ยวข้อง แต่งตั้งคณะกรรมการ/คณะทำงาน จัดทำ ร่างระเบียบกระทรวงสาธารณสุขเรื่อง การจัดบริการคลินิกพิเศษเฉพาะทางนอกเวลาราชการของหน่วยบริการ ในสังกัดกระทรวงสาธารณสุข พ.ศ. 2561 และทดลองนำร่างระเบียบฯ ดังกล่าว ไปใช้ในโรงพยาบาลนำร่อง 7 แห่ง เพื่อจัดทำผลสรุปข้อเสนอแนะเชิงนโยบายให้แก่ผู้บริหารของกระทรวงสาธารณสุข ข้อมูลจากโรงพยาบาลทั้ง 7 แห่ง พบว่า โดยเฉลี่ย ประชาชน (ผู้มารับบริการ) ร้อยละ 98.41 เจ้าหน้าที่ (ผู้ให้บริการ) ร้อยละ 96.77 และข้อมูลจากแบบสอบถาม ผู้บริหารในสังกัดกระทรวงสาธารณสุข ร้อยละ 95.36 “เห็นด้วย” ให้มีการจัดบริการคลินิกพิเศษเฉพาะทางนอกเวลาราชการ (Special Medical Clinic: SMC ) ต่อมาในวันที่ 12 กันยายน พ.ศ. 2561 กระทรวงสาธารณสุขได้ประกาศใช้ระเบียบฯ ดังกล่าว เป็นนโยบายเร่งด่วนให้หน่วยบริการในสังกัดกระทรวงสาธารณสุขดำเนินการต่อไป            This research is a descriptive study and used the qualitative method. The aims of this study were to study and to analyze the model of part- time services in health services unit, both inside and outside the Ministry of Public Health as well as laws and regulations associated to prepare policy recommendations for the development of part- time service systems and also used as information in the preparation of the Ministry of Public Health regulations. The secondary data was collected from research study report, forms of part- time clinical practice in service units, reviews of various and related laws and regulations. Also, an appointment of committee working group was made to draft the Ministry of Public Health regulations. Regarding the provision of special clinic services for non- official hours of service units under the Ministry of Public Health, 2018, there was an application of the regularity to the seven pilot hospitals for summarizing the policy suggestions to the exclusive holder. According to the seven hospitals data founded that there was “agree” to the Special Part-Time Medical Clinic services. The mean of rank was in agree as the Special MedicalClinic services including 98.41% of population (clients), professional (health providers) was 96.77% and 95. 41 of the exclusive holder in Ministry of Public Health. Moreover, The Ministry of Public Health was announced in emergency policy operation into the whole health unit in 12th September, 2018.
